Dermoid Cyst

A dermoid cyst is a collection of normal tissue in an abnormal space. They typically occur on the outer edge of the eyebrow but can be found on other parts of the eye or the body as well. The ones around the eye can be made of skin, hair, or fat. They are typically round, mobile (rolls under your fingers) and are filled with a cheesy material.

Dermoid cysts are typically found in the orbit, around the eyebrow. They can also be found on the white part of the eye (conjunctiva) and is called a limbal dermoid or epibulbar dermoid.

If the cyst is in a typical place around the eye, usually imaging is not necessary. However, if it is an unusual location, we can do imaging to rule out other causes of a cyst around the eye that may be more dangerous.

A dermoid cyst does not typically cause vision loss. However, if it breaks or ruptures it can cause an inflammatory reaction in the surrounding area and can lead to scarring and damage of normal eye structures around the cyst.

Treatment for Dermoid Cysts

To avoid the above-mentioned inflammatory reaction, we recommend doing a surgery to remove or excise the cyst in the operating room under general anesthesia. The cyst is also sent for biopsy and this way we can confirm the diagnosis. During the surgery, a small incision is made in the skin and the entire contents of the cyst are removed carefully. There is always a small scar after the surgery, and we will usually place the incision in the eyelid crease or under the eyebrow to make it as aesthetic as possible.

Every surgery has risks and benefits. The risks of this surgery are infection, bleeding, scarring and very minimal chance of recurrence. The likelihood of vision loss is extremely low, but any surgery on or around the eye has this risk.

During the first week after the surgery, you will come in for a post-operative visit where we will check the incision and ensure you are healing well. The skin tends to heal remarkably well on its own. If you choose not to elect for surgical removal of the cyst, we will monitor you annually.
