Revision Ptosis Surgery After Prior Eyelid Repair
This young man had previously undergone right upper eyelid ptosis surgery with another surgeon years earlier. Although the original procedure improved his eyelid position, he noticed some residual drooping of the right upper eyelid and was unhappy with the overall appearance of the eyelid.
On examination, the right upper eyelid was a few millimeters lower than the left and the crease poorly defined. Surgically, the goal was to improve the eyelid height and to create a more natural and symmetric upper eyelid crease.
To achieve these goals, I performed a revision ptosis repair using a Fasanella-Servat procedure combined with a small upper eyelid blepharoplasty. The surgery was designed to elevate the eyelid while refining the crease by removing a small amount of excess skin.
The postoperative photographs were taken approximately 6 weeks after surgery. The right upper eyelid demonstrates improved elevation and an improved upper eyelid crease. Overall eyelid symmetry and contour are improved, creating a more balanced appearance while maintaining a natural look.
Revision eyelid surgery can be more complex than primary surgery because the anatomy has been altered by prior procedures. Thoughtful planning is important.
