Before Surgery
Steve was brought to see Dr. Lichtenstein because of a slowly enlarging bump within his right upper eyelid. The mass had likely been present since early infancy but became increasingly noticeable as he grew.
On examination, the lesion was consistent with an orbital dermoid cyst, a common benign growth of the eye socket. Dermoid cysts are present from birth and form during early fetal development.
Although these cysts are not dangerous, they typically continue to enlarge slowly over time rather than disappearing on their own.
Why Surgery Is Often Recommended
One of the most common questions parents ask is: "If it's benign, why remove it?"
The answer is that dermoid cysts generally continue to grow throughout childhood. As they enlarge, they can become more noticeable and create fullness or asymmetry of the brow and upper eyelid.
In addition, the contents of a dermoid cyst can cause significant inflammation if the cyst ruptures, either spontaneously or after trauma. This inflammatory reaction can lead to scarring, contour irregularities, and make subsequent surgery more challenging.
For these reasons, many families elect to remove the cyst before problems develop.
Surgery
Steve underwent a brief outpatient procedure to remove the cyst.
An incision was placed within the natural upper eyelid crease, allowing the dermoid to be carefully dissected and removed in its entirety while protecting the surrounding tissues of the eyelid and orbit.
The surgery was completed without complication, and Steve returned home the same day.
Early Recovery
The postoperative photograph on the right was taken just five days after surgery.
Even at this early stage, the prominence caused by the dermoid cyst has already resolved, restoring a smoother contour to the brow. Some swelling and healing remain visible, which is completely expected during the first week after surgery.
Over the following weeks and months, the incision typically softens and becomes progressively less noticeable.
The Goal of Treatment
The goal of dermoid surgery is complete removal of the cyst before it becomes larger or causes problems.
For children like Steve, treatment offers a straightforward solution to a common congenital condition while restoring a natural appearance to the brow and surrounding tissues. This early postoperative photograph demonstrates the immediate improvement in contour that can be achieved following successful excision of an orbital dermoid cyst.
